Registration Dates:     Saturday, April 7th, April 21st, May 5th, May 19th and June 23rd

*please be aware, completing an online application DOES NOT REGISTER the child.

-For existing registered users, update grade level and any additional applicable information such as, address, phone #'s, etc...
- Fill out and PRINT the registration form. THIS FORM MUST BE WITH YOU AT THE TIME OF COMING IN TO FINALIZE YOUR REGISTRATION. WE WILL NOT HAVE COMPUTERS OR PRINTERS AVAILABLE.

-Athlete/Parent Code of Conduct
-Athletic Image Release
-Medical Form
-Copy of Birth Certificate
-Completed 2018 physical with a doctor's clearance (physicals must have been performed on or after 1/1/18)
-Copy of 2017 - 2018 Year end Report Card showing all quarters
**Every child will be required to register in their own age division

Fees:
Tackle- $250 / Flag- $175 / All Cheer- $250

Sibling Discount (3 or more):
Tackle- $225 / Flag- $150 / All Cheer- $225

*Minimum Fees Due at registration: Tackle/Cheer $100 & Flag $50
**Payment plans available and will be set up at registration
***Any payments using a debit/credit card will be charged a $5.00 surcharge fee
****Returned check fee (NFS) $25
